This Membership Agreement contains important information for you about Health Workers Hall of Fame (hwhalloffame.com™) and the terms and conditions of your participation in our association.
General Terms and Conditions:
All Members Sign Up and Sign On To Support One Collective Mission To:
1. Honor and memorialize the relentless efforts of Health Workers — as they strive to deliver health care services to all who need it.
2. Honor the all lives lost to COVID19. Especially, among three interdependent groups: Health Workers, Nursing Homes and Black and Brown communities who have been struck the hardest by COVID19.
3. Extinguish Burnout Syndrome among Health Workers and preserve the contributions and achievements of Unsung Heroes, — like those fighting COVID19 to ensure they no longer remain anonymous.
4. Resolve and mitigate Health Disparities among Black and Brown communities who are suffering disproportionately, — now and on the other side of COVID19.
5. Deliver a monumental public health response that is reflective of the need for emergency pandemic preparedness, to diminish the negative impact pandemics can have on the health of people, communities, our economy and way of life.
Also, to help The Health Workers Hall of Fame usher in a new era of national respect and recognition, of today’s Health Workers in clinical and non clinical job titles, while advancing their professional development.
Individual Membership Association Fee
- Be a voting member in good standing with Health Workers Hall of Fame Association: This mean:
- Make monthly membership payments of $25.00 per month
- Make semi-annual membership payments of $120.00 SAVE $25.00
- Make annual membership payment of $250.00 SAVE $50.00
Organizations Membership Association Fee
- Be a voting member in good standing with Health Workers Hall of Fame Association: This mean:

- Make semi-annual membership payments of $2000 SAVE $500.00

How to Cancel Your Membership
You may cancel your membership in the program at any time. To cancel your membership, simply contact us at info@hwhalloffame.com, or call the toll-free customer care number or send a letter requesting that your membership be cancelled to our corporate address listed above.
